We provide an outreach service to people who are referred to us by other healthcare professionals.

We provide an outreach service to people who are referred to us by other healthcare professionals.

We work closely with each person and the other people involved in their support to develop a care management plan.

Often, we provide the link between the person and the Community Mental Health Team.
A team of support workers runs our outreach service. It is managed by United Response.
They work with local Community Mental Health Teams.

We work with people in their own homes.

But we can support people before they go to hospital or after they come out of hospital.
We can help with things like everyday tasks around the house .
We can help with mental or physical health. Or we can give emotional help.
Practical help can be something like finding somewhere to live or help with managing money.
We can also help people with benefits.

Or we can go with someone to something they would like to do like the cinema or a local group.

We can help people with social and emotional things like making friends.

Or we can help people if they are stressed.

We do not control people's medicines. But we will tell the Care co-ordinator if someone is not taking their medicines.
We can also help to tell other people if there are problems with your medicines. Or if there are any other problems.
