Getting the right support

Carer’s Assessment – To request an assessment, contact Customer First on 0808 800 4005.  However, please bear in mind that you are not automatically eligible to receive the support identified in the assessment. Further information about the process is available from Carers UK.

Family Carer Emergency Planning Service – This is a County Registration scheme with a contingency plan to cover emergencies, where the cared for person is left without cover. To find out more contact Customer First on 0808 800 4005.

Assessment of needs – This aims to identify the care needs of the person you support. The care plan outlines a package of support, which can provide access to other services. Assessment does not automatically mean that a person is eligible to receive funding for statutory services. To find out more contact Customer First on 0808 800 4005.

Occupational Therapist Assessment – Identification of practical support needs and aids to support independence and mobility. To find out more contact Customer First on 0808 800 4005.

Travel and transport

People with a disability are eligible for concessionary bus passes. If unable to access public transport they may be entitled to travel vouchers. To find out more visit the Suffolk County Council website or call 0845 600 065.
